Type of inspection: Renewal An unannounced inspection was conducted on-site 02/13/2024 from 3:35 pm to 4:50 p.m. Remote review was conducted on 02/14/2024 to review documents forwarded by the facility. The Acknowledgement of Inspection form was signed and left at the facility on the date of the on-site inspection. A preliminary findings review was held on 02/15/2024. The exit interview was conducted on February 20, 2024 Number of youth: 7 Number of approved provider homes: 5 The licensing inspector completed a tour of the physical plant that included the office setting and conditions.
