Comments:

An unannounced monitoring inspection was conducted on 5/20/22. At the time of entrance ten participants were in care. Staff ratio's exceeded the required 1:6. The sample size consisted of four participants records and three staff records including a review of criminal. Participants were observed hydrating and engaging in activities . No participants required medication administration. No violation issued and exit interview held with Administrator.
