Date of Inspection: May 8, 2023 9am-3:30pm Type of Inspection: Monitoring inspection Census 14 Number of records reviewed and interviews conducted- 7 records, 3 interviews. All facility self-reported incidents since the last inspection were reviewed on this date. The LI observed participants participating in a variety of activity programs and eating lunch. Violations:

Standard #:	22VAC40-61-110-B
Description:	Based on record review and staff interview, it was determined that a staff member did not receive required orientation and training no later than three weeks after the staff members starting date of employment. Evidence: Staff B did not have a completed record of staff orientation and training on date of inspection.
Plan of Correction:	The staff orientation checklist will be completed and signed off within three weeks of the start date. Should the immediate supervisor not be available, a designee and or Divisional Director will be available to complete the process. The violation was reviewed, and all orientation checklist was reviewed and signed on May 23, 2023.
