Technical Assistance:
Please be sure the menu is posted for participants and the public to see in a conspicuous area. This visit your center is between food service contracts for the month of October. This center is in the process of working out the final details of contracting with another food services which should begin November 2019.

Comments:
Two licensing inspectors conducted an unannounced monitoring inspection at Pulaski Adult Day Service & Fall Prevention Center, Inc. on 10/01/2019. The inspection began at 10:04 am and concluded at 11:30 am. Participant files were reviewed, staff files were reviewed, and a tour of the building and grounds was conducted. The first aid kit was observed and reviewed. The center was found to have 8 participants on the day of the inspection. The required postings were observed. Interactions between staff and participants were observed. An exit meeting was held with designee of the administrator on 10/01/2019 and the findings were reviewed and at this time the opportunity was given to locate items that were not found in the files. One violation is being cited as a result of this monitoring inspection. Please develop a plan of correction for the violation cited and sign and date a copy of this notice and return to the licensing office within 10 days (10/14/2019) of receipt. Violations:
Standard #: 22VAC40-60-698-C
Description: Based on review of participant records, the center failed to maintain a list of all medications, including those taken at home and at the center for each participant at least twice a year.
EVIDENCE:
1. Participant #1 was enrolled for services on 04/04/2017 and had a medication list dated for 05/09/2018 for the 2018 year. There was a medication list provided dated 01/07/2019.
2. Participant #2 was enrolled for services on 04/30/2018 and the only medication lists available in the file was for 04/12/2018 and 01/07/2019.

Plan of Correction: The Director will request a current medication list for each participant at least twice a year. The director obtained and placed on the chart the current medication list for Participant #1 and Participant #2 on 10-2-19. [sic]
