Comments:
A monitoring inspection was initiated on 07/19/2021 and concluded on 07/23/2021. The administrator was contacted by telephone to initiate the inspection. The administrator reported that the current
census was 6. The inspector emailed the administrator a list of items required to
complete the remote documentation review portion of the inspection. The inspector
reviewed two resident records, two staff records, activities calendar, staff schedules, health and fire inspections submitted by the facility to ensure documentation was complete. The inspector conducted the on-site portion of the inspection on 07/22/2021. An exit interview was
conducted with the administrator on the date of inspection, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection.
Information gathered during the inspection determined non-compliance(s) with applicable
standards or law, and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-61-410-E
Description: Based upon observations made during the tour of the building, the center failed to ensure that all cleaning products or harmful materials be stored separately from food and be kept in a locked place when not in use.
EVIDENCE:
1. In the small area outside of the restrooms, behind the washing machine was a black caged area approximately 18 inches in height with a small lock on the outside. In the caged area was a bottle of Flo-Sure Premium Detergent Concentrate, laundry detergent with a tube traveling from the bottle of detergent into the washing machine detergent dispenser. The lock on the outside was properly secured, but the tubing piece was not installed properly into the apparatus that held the tubing into the bottle, therefore creating access to the liquid detergent, which was clearly marked "keep out of the reach of children", which indicates a hazardous material.

Plan of Correction: 1. The tubing for the washing machine was fixed during the inspection. The detergent container remains locked out of reach of participants. [sic]
