Comments:
On 11/1/2019 one inspector conducted a renewal study (10:30AM to 2:30PM). 16 participants were in care. Four participant records, three staff records, and one volunteer record were reviewed. There have been no new staff hired since the prior inspection. A meal and multiple activities were observed. The medications were checked (no medication pass during the inspection), and a physical plant tour was done.

Standard #: 22VAC40-60-570-B
Description: Based on participant record review, the facility failed to address some participant needs on Plans of Care (POC).

EVIDENCE:

1. The record for participant 2 shows an order for a "diabetic diet" was faxed in on 7/8/2019. This prescribed diet is not addressed on the POC, dated 8/19/2019.

2. The file for participant 3 shows this person is allergic to Cardura and PNCs [penicillins]. These medication allergies are not addressed on the POC, dated 5/20/2019.

Plan of Correction: 1. Updated Care Plan for Participant 2 on 11/8/19 to prescribed Diabetic Diet.

2. Updated POC for Participant 3 to include allergies to Cardura and PNCs (penicillin?s) on 11/8/19.

Will compare H& P to all additional supporting documentation to ensure they are correct moving forward.

Standard #: 22VAC40-60-698-D-2
Description: Based on observation, the facility failed to have a medication labeled with the participant's name.

EVIDENCE:

1. A tube of Abrevia was not labeled with a participant name. Staff identified it as belonging to participant 5, the only participant that uses this.

Plan of Correction: 1. Provided In-Service to RMA?s, LPN and Assistant Director to include DSS regulations on Medication Management and our policies and procedures about labeling medications on 11/6/19.
