Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 10/13/2020 and concluded on 10/20/2020. The director or in-charge person was contacted by telephone to initiate the inspection. There were ten participants present and three staff. The inspector emailed the director/person in charge a list of items required to complete the inspection. The Inspector reviewed two participant records and two staff records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliances with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the director/person in charge COVID-19 guidance posted on the ADCC webpage.

Violations:
Standard #: 22VAC40-61-230-D
Description: Based on participant record review, the facility failed to address some identified needs on the Plan of Care (POC).

EVIDENCE:

1. The assessment for participant 1 dated 8/5/2020 shows this resident needs human help with physical assistance with dressing after toileting/bathing, mobility (pushed by others), and is a fall risk. These are not addressed on the POC dated 8/5/2020.

2. The assessment for participant 2 dated 10/6/2020 shows this participant has difficulty chewing and this is not addressed on the POC dated 10/6/2020.

Plan of Correction: Updated care plans for participant 1 and 2.

Description: Based on participant record review, the facility failed to obtain a required signature on a plan of care (POC).

EVIDENCE:

1. The POC for participant 1 dated 8/5/2020 lacks a signature from the participant, family member, or legal representative.

2. The POC for participant 2 dated 10/6/2020 lacks a signature from the participant, family member, or legal representative.
