Comments:

An unannounced renewal inspection was conducted on 7-22-19 (ar 09:30/dep 12:00p). The census upon arrival was 6 participants. The director was present. A review of staff and participants records were conducted, emergency documents were reviewed, activity and meal posting were reviewed, water temperature was conducted with staff, and a tour of the facility was conducted. The first aid kit was reviewed for compliance, insurance information was reviewed, fire and health documents also reviewed. The participants were engaged in a morning activity (daily chronicle and news and "I remember when"). Participants and staff scheduled for an outing after an early lunch, all staff, participants and volunteers escorts. The acknowledgement form was signed following the exit by the director. Comment: LI suggested director have participant/legal representative sign and date when changes in services or any payment changes occur as this changes the original agreement for services. According to director, facility is considering taking medicaid participants in the future.
