Comments:

A renewal inspection was initiated on August 10, 2021 and concluded on August 20, 2021. The director was contacted by telephone to initiate the inspection. The director reported that the current census was 12. The inspector emailed the director a list of items required to complete the remote documentation review portion of the inspection. The inspector reviewed two participant records, two staff records, activities calendar, menu, staff schedule, fire drills, health and fire inspection submitted by the facility to ensure documentation was complete. The inspector conducted the on-site portion of the inspection on August 19, 2021. An exit interview was conducted with the Director on August 20, 2021, where findings were reviewed and an opportunity was given for questions, as well as for providing any information or documentation which was not available during the inspection. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations were documented on the violation notice issued to the facility.

Violations:

Standard #:	22VAC40-61-150-A-3
Description:	Based on record review and staff interview, the facility failed to ensure when adults with mental impairments participate at the center, at least four of the required hours shall focus on topics related to participants' mental impairments. Evidence: 1. Staff #3's training record documented two of the required fours hours of annual mental health training hours. Staff's date of hire was noted as 3-1-20. 2. On 8-20-21, during exit interview, staff #1 acknowledged staff #3 did not have all required mental health training hours.
Plan of Correction:	Staff #3 will complete 2 additional hours of mental health training. Also an audit of all staff training will be conducted to make sure all adult day staff have the required training hours per 22VAC40-61-(3) 150-A.3
