Comments:

One licensing inspector conducted an announced mandated initial inspection at the Marion Adult Daycare Center on 02/16/2022. The inspection began at 1:28 pm and concluded at 2:07 pm. During the inspection the physical plant, the first aid kit, and emergency food and water supplies were observed. An exit interview was held with the center director and other key staff on the day of inspection. As a result of this inspection one violation is being cited. Please complete the "description of action to be taken" and "date to be corrected" for each violation cited on the violation notice, and then return the signed and dated copy to the licensing office within 10 calendar days (03/05/2022) of receipt. Description:	Based on observations made during the tour of the physical plant, the facility failed to ensure all fixtures are in good repair. EVIDENCE: 1. In the restroom containing the shower there was a water leak observed coming from the ceiling and dripping onto the floor. The ceiling panel was also missing. The facility representative reported the pipe in the ceiling was leaking and in need of repair.
Plan of Correction:	Water leak in ceiling of ADC bathroom to be repaired by construction staff. Ceiling panel to be replaced when leak is repaired. [sic]
