Comments:

An unannounced monitoring inspection was conducted on 1-24-22 (arrival 08:40 am/dep 12:00 pm) by two Licensing Inspectors. The census was 14 upon arrival. A tour of the facility was conducted, required posting were observed, am/pm snack and lunch menu posted, building temperature observed, records were reviewed, water temperature observed in two restrooms, activities and a med pass were observed. Staff and participant files were reviewed, including fire drills. The acknowledgement form was emailed to the director, who was present upon LI's arrival. There were no violations cited.
