Comments:
This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated on 6/1/2020 and concluded on 6/4/2020. The office manager was contacted by telephone to initiate the inspection. The office manager reported that the current census was eleven. The inspector emailed the office manager a list of items required to complete the inspection. The inspector reviewed two (2) resident records, two (2) staff records, fire and health inspection reports, physician orders, pharmacy review, emergency evacuation drills, staff schedules, etc. submitted by the facility to ensure documentation was complete.

No new staff hired since last inspection.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the facility.

Please complete the "plan of correction" and "date to be corrected" for the violation cited on the violation notice and return to the Inspector within 10 days. You will need to specify how the deficient practice will be or has been corrected. Just writing the word "corrected" is not acceptable. Your plan of correction must contain: 1) steps to correct the non-compliance with the standard, 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure.

Violations:
Standard #: 22VAC40-73-680-D
Description: Based on a desk review for two residents, medications were not administered in accordance with the physician's or other prescriber's instructions and consistent with the standards of practice outlined in the current registered medication aide curriculum approved by the Virginia Board of Nursing.

Evidence: A review of the current physician orders for two residents (# 1 and # 2) found orders for Vitamin C 500 mg twice a day. The administration of Vitamin C was not documented on the medication administration records provided during the desk review.

Plan of Correction: The facility manager will check the MAR's against the physician's orders to ensure all medications are documented as directed. 06/04/20
