Comments:

An unannounced monitoring inspection was initiated by the Licensing Inspector on October 4, 2019 from 9:35 a.m. to 11:35 a.m. and a follow-up visit was conducted on October 11, 2019 from 8:00 a.m. to 9:00 a.m. to complete the inspection. A census of 8 residents was reported. Previous violations were reviewed and corrected. The following items were reviewed/observed during the inspection: a tour of the facility, a sample of 4 resident and 3 staff records, facility documentation, first aid kit and emergency food and water supplies, medication pass observation/physician's orders/Medication Administration Records (MARs), and facility postings.
