Comments:

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia. A complaint inspection was initiated on 5/20/2021 and concluded on 7/9/2021. A complaint was received by the department regarding allegations in the areas of resident care, building and grounds. The Administrator was contacted by telephone to conduct the investigation. The licensing inspector emailed the Administrator a list of documentation required to complete the investigation. The evidence gathered during the investigation supported the allegation of non-compliance with standards or law, and violations were issued. Any violations not related to the complaint but identified during the course of the investigation can be found on the violation notice.

Description:	Based on a virtual inspection conducted on 5/20/2021, the facility failed to keep all furnishings and fixtures clean. EVIDENCE: 1. Room R2 and B27 were noted to have a dirty toilets on the day of the virtual inspection.
Plan of Correction:	The Administrator has set up rounds 4 times a day with housekeeping and management on these bathrooms to ensure that the toilets are clean. Direct care staff are also checking the resident bathrooms each time they are in the rooms assisting with ADLs to ensure that the toilet are clean.
