An unannounced complaint inspection was completed on October 03, 2019, from 2:45 p.m. to 3:29 p.m. for Jan's Residential Home I in Blackstone, Virginia. Upon arrival, the assigned inspector met with the designated staff member on duty regarding buildings and grounds allegations. Specific items were reviewed during the building tour based upon the nature of the complaint. The complaint is valid. Please submit a written plan of correction within the next ten calendar days. Your plan should specify how you will correct each violation, an intended plan for future compliance, a date of correction for each violation and the job title of staff responsible for implementing and monitoring preventive measures.
