Type of inspection: Other Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: 4-18-23 from 10:00 a.m.-10:30 a.m. The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection. Number of residents present at the facility at the beginning of the inspection: 62 Additional Comments/Discussion: The licensing inspector toured the facility?s converted memory care rooms and measurements were taken. The facility is not requesting a change in their licensed capacity. An exit meeting was conducted to review the inspection findings.
