Comments:

This inspection was conducted by licensing staff using an alternate remote protocol necessary due to a state of emergency health pandemic declared by the Governor of Virginia. An inspection was initiated on 9/22/20 to follow-up on a self -reported incident. The inspector reviewed staff schedules, an incident report, the resident's service plan and physical examination submitted by the facility . Information gathered during the inspection determined non-compliances with applicable standards or law, and the violations are documented on the violation notice issued to the facility.

Standard #:	22VAC40-73-70-A
Description:	Based on a review of a facility's incident report and an email received on 9/21/2020, the facility failed to report an incident that negatively threatened the health, safety and welfare of a resident in care to the regional licensing office within 24 hours. Evidence: The licensing inspector was notified on 9/21/2020 of the elopement of a resident from the secure memory care unit. It was documented in the incident report received on 9/21/20 documented that resident # 1 "was found at about 200 meters away from the facility at a traffic light". The incident occurred on 9/15/20, the incident was not reported to the licensing office until 9/22/20.

Description:	Based on an incident report and email received 9/21/20, the facility failed to provide supervision of a resident 's care and activities, including attention to specialized needs, such as prevention of wandering from the premises. Evidence: It was reported by the facility on 9/21/20 that a resident eloped from the secure memory care environment and was found at about 200 meters away from the facility at a traffic light".
