Comments:
Licensing Inspector (LI) conducted unannounced complaint investigation on 7/19/19 regarding building maintenance specific to an elevator not working and dirty carpets. LI observed nine hallways, two stairwells, two elevators and 25 resident rooms. LI interviewed eight residents, one family member and management staff. Complaint regarding Standard 870.A is deemed valid as a preponderance of evidence gathered during the investigation supported the allegations. Exit interview conducted with Acknowledgment of Inspection provided on 7/19/2019 with Associate Executive Director, Health and Wellness Director and Business Office Manager and with Executive Director by phone. The violation notice regarding the standard was deemed valid and inspection paperwork emailed to Executive Director. Areas of non-compliance are identified on the violation notice. Please complete the "plan of correction" and "date to be corrected" for each violation cited on the violation notice and return to the licensing office within 10 calendar days. Please specify how the deficient practice will be or has been corrected. Just writing the word "corrected" is not acceptable. The plan of correction must contain: 1) steps to correct the non-compliance with the standard(s), 2) measures to prevent the non-compliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s). Description: Based on observation and interview, facility failed to ensure that the interior of all buildings shall be maintained in good repair and kept clean. Evidence: 1/2 elevators is currently being repaired with signs posted to use the second elevator; and carpets in 6/9 hallways observed appear dirty and carpets in 15/25 resident rooms observed appear dirty and worn. Carpeted areas in hallways on floors 2, 5, 6, 7, 8 and 9 and resident room numbers 209, 213, 315, 317, 411-A, 411-B, 413, 507, 614, 631, 635, 636, 718, 814 and 913 appear dirty and worn.

Plan of Correction: Signs were reposted regarding elevator re-modification. Carpeting on every floor were professionally cleaned by 7/29/2019. An increased cleaning scheduled is implemented during Re-modification project due to extra traffic. All identified resident rooms carpets will be cleaned and then resident rooms will be monitored weekly and subsequent cleanings will be scheduled. 7/29/2019 and ongoing.
