Comments:

Licensing Inspector (LI) conducted unannounced complaint investigation on 02/25/2019 regarding functioning elevator and generator, resident communication and available food service during a countywide power outage documented on 1/24/2020. LI reviewed emergency management plan and reviews, inspections and other documentation. While the preponderance of evidence gathered during the investigation did not support the allegations included in the complaint, the complaint is deemed valid as a violation related to Standard 22VAC40-73-950.E was found and cited. Violation and risk rating discussed during exit interview conducted with Administrator and Maintenance Director. Areas of non-compliance are identified on the violation notice. Please complete the "plan of correction" and "date to be corrected" for each violation cited on the violation notice and return to the licensing office within 10 calendar days. Please specify how the deficient practice will be or has been corrected. Just writing the word "corrected" is not acceptable. The plan of correction must contain: 1) steps to correct the non-compliance with the standard(s), 2) measures to prevent the non-compliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s). Description:	Based on documentation, facility failed to ensure that the facility shall implement a semi-annual review on the emergency preparedness and response plan for all staff, residents and volunteers. Evidence: The facility reviews of the emergency preparedness and response plan documented on 1/7/2020 and on 10/17/2019 did not include signatures of all staff, residents and volunteers.
Plan of Correction:	On 2/27/2020 the community repeated the semi-annual review of emergency preparedness and response plan with the remaining staff and residents that did not attend the original training. Future Emergency preparedness plans will be reviewed and signed by ALL staff and residents. ED or designee will audit training logs on 3/20/20 to verify all signatures and dates are present. Executive Director or designee will audit 100% of training logs quarterly to ensure continued compliance.
