Comments:
This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A complaint inspection was initiated on October 14, 2020, conducted on October 21, 2020 and concluded on October 22, 2020. A complaint was received by the department regarding allegations in the areas of Resident Care and Related Services. The Administrator was contacted by telephone to conduct the investigation. The licensing inspector emailed the Administrator a list of documentation required to complete the investigation. Topics discussed during the investigation included Admission, Retention, and Discharge of Residents, Staff Training, and Medication Administration.

The evidence gathered during the investigation supported the allegation of non-compliance with standards or law, and violations were issued. Any violations not related to the complaint but identified during the course of the investigation can be found on the violation notice.

Description: Based on record review and interview, the facility failed to adhere to the medication management plan regarding methods to ensure that each resident's prescription medications were ordered for the resident and refilled in a timely manner to avoid missed dosages.

Evidence:

1. Resident #1?s August, September, and October Medication Administration Records (MARs) documented the following medications were not administered:

A. Carvedilol 12.5 mg ? 08-15-20 at 9 p.m.,

B. Dok tab 100mg ? 08-01-20, 08-19-20, 08-25-20, 08-26-20, 08-27-20, and 10-12-20 at 8 a.m.; 08-15-20, 09-13-20, and 09-14-20. at 9 p.m.,

C. Risperidone 2 mg ? 08-01-20 at 5 p.m.,

D. Clonazepam 0.5 mg ? 09-07-20, 09-08-20, 09-10-20, 09-12-20, 09-13-20, 09-14-20, and 09-15-20 at 9 p.m.,

E. Ferrous Sulfate 325 mg ? 10-12-20 at 8 a.m.,

F. Loratadine 10 mg ? 10-12-20 at 8 a.m.; and

G. Omeprazole 20 mg ? 10-11-20 and 10-12-20 at 6 a.m.

2. Staff #1 confirmed medications were not available onsite, and that the facility failed to adhere to the medication management plan regarding methods to ensure that each resident's prescription medications were ordered for the resident and refilled in a timely manner to avoid missed dosages.

Plan of Correction: All medications have been requested to be put on 90 day refill other than one narcotic which must be on 30 day refill. We must call the VA Hospital every five days after ordering to monitor the refills. In the event we get to a five day supply of any medications, it will be called in to our LTC Pharmacy for a seven day supply. This will be repeated until the required medications have arrived from the VA Hospital in Hampton. This will be the process going forward so that all medications will be given to said resident or any other VA clients we have. This process began on 10/30/20.
