Subjectivity

Comments:
An announced inspection was conducted on 3/31/2022 from 10:11 am to 11:10 am from a Licensing Inspector to review the newly renovated safe, secure environment. The unit is intended to hold up to ten residents. A tour of the space was completed to include a secure courtyard, living area, and dining area. The activity calendar specific to the unit, fire evacuation plan, and resident rights were posted. Five apartments for double occupancy, previously utilized as assisted living beds, were observed and measured.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law, and violations were documented on the violation notice issued to the facility.

Violations:
Standard #: 22VAC40-73-1150-A
Description: Based on observation and interview, the facility failed to ensure doors that lead to unprotected areas be monitored or secured through devices that conform to applicable building and fire codes, including door alarms, cameras, constant staff oversight, security bracelets that are part of an alarm system, pressure pads at doorways, delayed egress mechanisms, locking devices, or perimeter fence gates for residents residing in a safe, secure environment.

Evidence:

1. On 3/31/22, while touring the safe, secure environment, the two doors were not secured from the inside of the unit. The two doors are enter and exit points from inside the safe, secure environment on both ends of the hallway. Both doors were secured upon entry into the safe, secure environment via badge access; however, from inside the unit, one would be able to exit the unit without a locking or monitoring device/mechanism.

2. Staff #1, Staff #2 and Staff #3 acknowledged the two exits of the safe, secure environment were not secured.

Plan of Correction: Johnson Controls will be here to complete the installation of the door security system on April 11, 2022. If at the end of the day on April 11, 2022 the badge swipe is non-functioning we will have the key pad lock reinstalled by close of business. This will correct the violation issued on 3/31/22. There are currently no residents residing in this area due to license modification not being completed at this time.
