The LI for Town Creek conducted an unannounced investigation via phone in regards to a facility self reported incident on 3/31/2020. A follow up email was received on 4/13/2020. The facility self reported incident form, other facility documentation and local Adult Protective Services documentation was reviewed and phone interviews were conducted with staff.
