A non-mandated monitoring inspection was initiated on August 23, 2021 and concluded on September 1, 2021. A self-reported incident was received by the department regarding allegations in the areas of resident care and related services. The Associate ED was contacted by telephone to conduct the investigation. The licensing inspector emailed the Associate ED, Executive Director, and Resident Care Director a list of documentation required to complete the investigation.
The evidence gathered during the investigation supported the allegation of non-compliance with standards or law, and violations were issued.

Standard #: 22VAC40-73-130-A Description: Based on record review and interview with staff, the facility failed to ensure all staff who are mandated reporters under ? 63.2-1606 of the Code of Virginia shall report suspected abuse, neglect, or exploitation of residents in accordance with that section.
Evidence:
1. An incident report received by the regional licensing office on 7-10-2021 documented, ?Resident?s [#1] Left wrist and arm was noted to be swollen when resident woke up. Arm was tender to touch?. Xray showed wrist fracture. Unknown how or when injury happened. Resident is unable to recall any injury/fall??
2. Per ? 63.2-1606 of the Code of Virginia, ?Matters giving reason to suspect the abuse, neglect or exploitation of adults shall be reported immediately upon the reporting person's determination that there is such reason to suspect.? The facility could not rule out suspected abuse based on not knowing what occurred.
3. Staff #1 confirmed during interview that the incident was not reported to the local adult protective services despite injury of unknown origin.Plan of Correction: Not available online. Contact Inspector for more information.
