22VAC40-73 BUILDING AND GROUNDS

Comments:
An announced modification inspection was conducted on 1-20-22. (AR 09:00 a.m./ Dep 09:45 a.m.) A tour of Buildings B and C was conducted with the Administrator and Director of Maintenance. Violations were reviewed with staff. The modification process and the current conditional license process was also reviewed. Areas on non-compliance found were reviewed during the inspection with the administrator.
The Acknowledgement Form was sent to the administrator via email on 1-20-22.
Please complete the "plan of correction" and "date to be corrected" for each violation cited on the violation notice and return to me within 10 calendar days from today. Your plan of correction must contain: 1. Steps to correct the noncompliance with the standard(s) 2. Measures to prevent the noncompliance from occurring again 3. Person(s) responsible for implementing each step and/or monitoring any preventative measure(s). Violations:
Standard #: 22VAC40-73-870-A
Description: Based on observation and staff interview, the facility failed to ensure the interior of the building was maintained in good repair and kept clean and free of rubbish.

Evidence:
1. On 1-20-22 during a tour of the facility with staffs #1 and #2, the ceiling lights in rooms # 1 and #2 in building B were observed to have grey colored particles in the globe portion of the ceiling lights.
2. Staff #2 stated the items were bugs that were coming in from through the heat and air conditioning unit in the room.
3. Staff #1 and #2 acknowledged the presence of bugs in the ceiling lights during the tour.

Plan of Correction: The Maintenance Director, Administrator or Designee will ensure that the interior of the building is maintained in good repair and kept clean and free of rubbish.
1. The Maintenance Director or designee will clean the ceiling lights in building B by 1/28/22. The Maintenance Director is coordinating with Ecolab to establish a plan to eliminate any future issues with bugs entering the rooms through the PTAC from the mulch outside of the individual rooms. The Maintenance Director and Administrator will perform a walkthrough of the building monthly to ensure compliance for 3 months and intermittently moving forward.

Description: Based on observation and staff interview, the facility failed to ensure the signaling device was visible and or/audible in a manner that permits staff to determine the origin of the signal.

Evidence:
1. On 1-20-22 during a tour of building B with staffs #1 and #2, when the inspector pulled the call bell in the bathroom of room #1 in building B, staff #1 was asked if staff would respond.
2. Staff #1 stated, no one would respond because the signaling device was being worked.
3. Staff #1 and #2 acknowledged the signaling devices in Building B and C, that was being inspected for licensing was not operation during the inspection.

Plan of Correction: The Maintenance Director, Administrator or Designee will ensure the signaling device is visible and or audible in a manner to determine the origin of the signal.
1. The signaling devices in building B and C were tested and operational prior to the end of the day on 1/20/22. The Maintenance Director, Administrator, or Designee will conduct random audits of the signaling devices monthly for 3 months or as needed moving forward to ensure compliance.
