Technical Assistance:

LI and administrator discussed the process for enrolling in the Phase II training.

Comments:

Licensing Inspector (LI) conducted announced initial inspection on 1/10/2022. LI walked the physical plant, verified window and room measurements, and reviewed policies and procedures. The Fire and Health Inspections have been completed and the Certificate of Occupancy has been granted. The inspection summary and supplemental form were issued via email. No violations were cited.
