Comments:

Type of inspection: Other Date(s) of inspection and time the licensing inspector was on-site at the facility for each day of the inspection: LI entered the facility at 11:10 am on 4/11/2023 and exited at 1:45 pm on 4/11/2023. The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection. The regional licensing office was notified by the facility of plans to make 20 beds currently being used for skilled nursing into beds for ALF residents. LI received a floor plan. The licensing inspector completed a tour of the physical plant that included the building and grounds of the facility. Number of resident records reviewed: 0 Number of staff records reviewed: 0 Number of interviews conducted with residents: 0 Number of interviews conducted with staff: 0 Observations by licensing inspector: LI measured the rooms that are to be used for ALF residents and also inspected the rooms for call bell systems. Additional Comments/Discussion: An exit meeting will be conducted to review the inspection findings.
