Type of inspection: Initial
Date(s) of inspection and time the licensing inspector completed inspection virtually for the facility: 12/09/2022
Number of residents present at the facility at the beginning of the inspection: 33
The licensing inspector completed a virtual tour of the physical plant that included the building and grounds of the facility. The facility recently turned the lower level into a memory care unit. This was reviewed at a previous inspection under the current owners.

Observations by licensing inspector: This initial inspection was completed virtually due to Covid being in the building. The facility had recently had an inspection under the current licensee that covered many of the areas of review. The administrator will remain and is aware that any previous violations should be addressed by the new management company when the changeover occurs. Licensing is still awaiting a current fire and health inspection which should be forthcoming after the first of the year. Copies will be sent to the licensing office to be added to the application. Background checks related to the new management company are also in process and will be forwarded upon receipt. In addition to the previous violation issues, the facility will need to secure new background checks for all employees, update all employee information to reflect new employers and complete a new disclosure and admission agreements with the residents. The facility will be issued a conditional license that is good for six months. At the end of the six months the facility will complete a renewal application. Please note no fee is required for moving from conditional to a regular license.
Additional Comments/Discussion: The health department continues to issue health certificates as opposed to completing full inspections unless there is a complaint due to remaining in a pandemic status.
An exit meeting will be conducted to review the inspection findings.
