Technical Assistance:
1) Technical assistance provided during the December 28, 2022, initial inspection, concerning the following: Buildings/ Grounds and Personnel in regards to standards: 870 A and 260 A/260 C.

Comments:
Type of inspection: Initial
Date(s) of inspection: 12/ 20/2022 and 12/28/2022
Time the licensing inspector was on-site at the facility for each day of the inspection:
The Acknowledgement of Inspection form was signed and left at the facility for each date of the inspection.

Number of residents present at the facility at the beginning of the inspection: Twenty-two- (22)
The licensing inspector completed a tour of the physical plant that included the building and grounds of the facility.
Number of resident records reviewed: Six- (6)
Number of staff records reviewed: Five- (5)
Number of interviews conducted with residents: Interviews were conducted during medication pass observation and the building tours.
Number of interviews conducted with staff: Three- (3)
Observations by licensing inspector: Buildings/Grounds, medication administration observation, resident direct care services, 96-hour emergency foods and record reviews.
Additional Comments/Discussion:
The first initial inspection was attempted on December 20, 2022, but the inspection was rescheduled due to the facility?s quarantine status. Upon the facility?s reduction of COVID-19 cases another initial inspection was scheduled for December 28, 2022. Technical assistance provided during the December 28, 2022, initial inspection, concerning the following:
Buildings/ Grounds and Personnel.

An exit meeting was conducted to review the inspection findings. The evidence gathered during the inspection determined no violations with applicable standard(s) or law. The inspection summary will be posted to the VDSS website should the facility be issued a license to operate. The department's inspection findings are subject to public disclosure.
