Comments:
An unannounced inspection was conducted at the facility to investigate allegations regarding facility upkeep and cleanliness, resident care and hot water temperatures. The allegations were determined to be valid, violations were cited. Please complete the "plan of correction" and "date to be corrected" for the violations cited on the violation notice and return to the Inspector within 10 days. You will need to specify how the deficient practice will be or has been corrected. Just writing the word "corrected" is not acceptable. Your plan of correction must contain: 1) steps to correct the non-compliance with the standard, 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s). Description: Based on a review of the individualized service plans(ISP) and care logs for three residents by three licensing representatives on 8/22/2019, it was determined that the facility did not provide care interventions as indicated on one residents' care logs.

Evidence:
1. Resident # 1 - (1) Staff care logs state "check resident every 2-3 hours in apartment when awake to offer any needed assistance." Staff care logs documented that resident was checked at 6:01, 6:02, 13:03, 13:04, 13:05. The time between checks is more than three hours.

Plan of Correction: 1. Additional care logs for resident #1 was provided to licensing inspector as followup to the initial inspection. The care logs provided show resident #1 has been checked on every two hours beginning 8/1/2019 through the date of the inspection on 8/22/2019.
2. Resident # 1 continues to be checked on every two hours and as needed by assigned direct care staff.
3. DON or designee will audit care logs for resident # 1 5 days a week to ensure they are being documented.

Description: Based on an inspection of the building/resident rooms on 8/22/2019, hot water taps are not being maintained at temperatures within the range of 105F and 120F.

Evidence: Hot water temperatures were measured by two licensing representatives in a random sampling of resident rooms using a portable thermostat. The temperatures at the hot water taps in rooms 110, 113, and 133 were measured at less than 105F.

Plan of Correction: 1.Preventative maintenance was taking place at time of inspection on the hot water system. After preventative maintenance was complete, hot water temperatures were reading within required range og 105F and 120F.
2. Environmental Services Director or designee will notify facility staff and residents of all required preventative maintenance. Staff will provide accommodations to residents as needed.
3. Any repairs or followup identified during preventative maintenance will be reported to the Executive Director by Environmental Services Director or designee. Executive Director will ensure item/items are repaired and/or replaced.

Description: Based on an inspection of the building on 8/22/2019 by two licensing representatives, the building is not being kept clean and in good repair.

Evidence: A random inspection of the building found carpet stains/stredded carpet in room 239, dirt tracks in the carpet in room 113, soiled carpet/soiled floors in hallways on the 2nd and thrid floors. (photos taken)

Plan of Correction: 1. Carpets in 39, 113, 2nd floor and 3rd floor hallways have been cleaned by housekeeping.
2. A weekly carpet cleaning schedule of all common areas has been established by housekeeping staff.
3. Residents are encouraged to notify housekeeping or designee when they see a stain in their carpet or feel their carpet needs to be cleaned. Housekeeping cleans carpet in resident's apartments when requested and/or stains are identified.
