A monitoring inspection was conducted on March 5, 2024 with the center director and staff. There were 48 children present, ranging in ages from 6 months to 12 years, with 9 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 6 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard #: 22.1-289.035-B-4 Description: Based on review of six staff records, the center failed to ensure that out of state background checks were conducted as required. Evidence: Staff 1 (date of employment 1/08/2024) indicated on the sworn disclosure statement dated 1/08/2024 that they had lived outside the state of Virginia within the past five years. There was no documentation to support that the required out of state search of the child abuse and neglect registry was submitted within 30 days as required. Plan of Correction: Center director will conduct a search of the central registry.
Standard #: 8VAC20-780-240-B Description: Based on review of six staff records, the facility failed to have documentation of orientation prior to working alone with children. Evidence: Staff 4 (date of employment 6/15/2023) did not have documentation of orientation on file. Plan of Correction: Center director will provide orientation training for Staff 4.
Standard #: 8VAC20-780-340-D Description: Based on review of six staff records, the center failed to ensure at least one staff in each grouping of children meets lead teacher qualifications. Evidence: Staff 4 was alone with a
group of children. There was no documentation on file to support that Staff 4 meets lead teacher qualifications.Plan of Correction: Staff 4 was substituting for a staff person who was absent.
