A Subsidy Health and Safety Inspection (SHSI) was conducted on 7/16/2024 from 1:05pm to 4:30pm. A licensing inspection was conducted on the same date as the SHSI inspection. The two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements not covered in licensing standards were reviewed during this supplemental inspection. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed during the exit meeting.

Standard #: 8VAC20-790-600-F Description: Based on record review the vendor did not ensure that each staff complete the department?s health and safety update course.
Evidence:
1.The records for Staff #1 (hired in 2021), Staff #2 (hired in 2020), and Staff #3 (hired in 2010) did not contain documentation that the department?s health and safety update course had been completed in 2023.
