Comments:

An unannounced monitoring inspection was conducted 5/20/2024 from approximately 9:45am-12:00pm. There were 39 children directly supervised by 10 staff in four classrooms divided by age, ratios were in compliance.. The physical plant, 4 staff records, 4 children?s records, medications, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed on the playground, participating in circle time, signing and discussing exercising, and coloring. There was an assortment of age appropriate materials for the children in care. Areas of non-compliance are identified in the Violation Notice. Standard #:	8VAC20-780-150-B
Description:	The center did not obtain immunization record for children that includes the date of the physical examination and dates immunizations were received and shall be signed by a physician, his designee, or an official of a local health department. Evidence: Child #3's record of immunizations did not have signature of doctor.
Plan of Correction:	Director to request immunization record be complete with all required information for child #3 and children prior to attending.
Standard #:	8VAC20-780-70
Description:	Based on record review and interview, the center did not have documentation that two or more references as to character and reputation as well as competency were checked before employment. Evidence: 1. Staff #1 hired 2/1/24 reference 5/10/24. 2. Staff #2 hired 2/27/24 references 2/28/24 and 3/6/24
Plan of Correction:	Director to complete at least two references prior to star date for new hires.

Description:	Based on record review and interview, the facility did not practice lockdown drill at least annually. Evidence: 1. Drill record showed date of last lockdown drill was 3/24/23. Director stated no lockdown drill has occurred since 3/24/23.
Plan of Correction:	Director to conduct lockdown drill.

Description:	Based on observation the facility did not have the findings of the most recent inspection of the facility in a prominent place at each public entrance of the licensed premises to advise consumers of serious or persistent violations. Evidence: The facility did not have the most current inspection notice posted or available at this inspection.
