Comments:
An unannounced monitoring inspection was conducted on 5/3/24 from 10:10am to 12:25pm. At the time of the inspection there were 58 children in care and 11 staff members. Children were observed participating in various activities including centers, story time and lunch. Records were reviewed for four children and six staff members. Medication, evacuation drills, emergency supplies and other required records and postings were also reviewed. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed during the exit meeting.

Violations:
Standard #: 8VAC20-770-60-C-2
Description: Based on record review the center did not ensure that all staff have a central registry finding within 30 days of employment.
Evidence:
1.Staff #1 (date of hire 10/20/23) was missing documentation of a central registry search.
2.Staff #1 was observed working during the time of the inspection.

Plan of Correction: Central Registry had been submitted for Staff 1 but was not in the employee?s file. Staff 1 no longer works at this center but results have been added to the file. Moving forward, Center Management will follow up on Central Registry results if we have not received within 30 days and will ensure results are placed in the employee file once we receive.

Standard #: 8VAC20-780-270-A
Description: Repeat Violation
Based on observation the center did not ensure that areas are maintained in a safe condition.
Evidence:
1.On the preschool playground, the poles of a swing set have areas of peeling paint.
2.The bottom of a wire fence between the infant/toddler and preschool playgrounds had broken wiring that was sharp and jagged.

Plan of Correction: Work Orders have been placed for the two playground concerns. Center Director and District Manager have escalated these issues to the Facilities Manager and Compliance Manager to get them addressed as soon as possible. Center Management will continue to do Playground Safety Checks and will submit Work Orders for any new issues that arise.

Standard #: 8VAC20-780-280-B
Description: Based on observation the center did not ensure that hazardous substances such as cleaning materials are kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1.There was a spray bottle of bleach and water solution left unlocked by the diaper changing area in the infant two classroom.

Plan of Correction: Staff will have a refresher training at the next Staff Meeting scheduled on 5/23 to review the expectation of keeping all hazardous substances locked away at all times. Center Management will check that hazardous substances are being kept locked away whenever they are stepping into classrooms.

Standard #: 8VAC20-780-570-A
Description: Based on observation and interview the center did not ensure that when a child is placed in a high chair, the protective belt is fastened securely.
Evidence:
1. There were three infants observed eating in high chairs with protective belts left unfastened.
2.Classroom staff confirmed that the protective belts were not fastened.

Plan of Correction: Staff will have a refresher training at the next Staff Meeting scheduled on 5/23 to review the expectation of using the protective belts whenever the high chairs are being used. Center Management will check that protective belts are being used whenever they are observing the Infant classrooms.
