Technical Assistance:
Technical assistance was provided and licensing standards regarding injury reports, step down trash can for diapering, cleaning materials with food storage, first aid bags on the bus and hire dates.

Comments:
An unannounced monitoring inspection was conducted on April 23, 2024 from 9:50am to 1:05pm. At the time of the inspection there were 84 children in care and 12 staff members. Children were observed participating in various activities including story time, center play, handwashing, lunch, and nap. Records were reviewed for five children and five staff members. Evacuation drills, emergency supplies and other required records and postings were reviewed. The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice and discussed with the director during the exit interview.

Violations:
Standard #: 8VAC20-780-140-A
Description: Based on record review, the center did not ensure that each child shall have a physical examination by or under the direction of a physician before the child's attendance or within 30 days after the first day of attendance.
Evidence:
1. There was no documentation of a physical examination for child #6 enrolled on 9/5/2023.
2. Staff #1 confirmed that the physical examination was not in the child?s record.

Plan of Correction: Gave family until 5/24 to provide an up-to-date physical for the child.
The administration will ensure all children have an up-to-date physical on record before the child?s start date.

Standard #: 8VAC20-780-160-A
Description: Based on record review, the center did not ensure that a tuberculosis screening had been completed within the last 30 calendar days of the date of employment and be signed by a physician, physician's designee, or an official of the local health department.
Evidence:
1. The TB screening on file for staff #2 was completed on 10/02/2023 which is more than 30 days prior to the hire date of 01/24/2024.
2. Staff #1 confirmed that the date of the screening was more than 30 days.

Plan of Correction: Administration will ensure all staff have all required completed documentation prior to the start of employment.

Standard #: 8VAC20-780-60-A
Description: Based on record review the center did not ensure that they maintain and keep at the center a separate record for each child enrolled with the required information.
Evidence:
1. The record for child #1 (enrolled 5/17/19) did not have documentation of an annual update signature from a parent/guardian.
2. The record for child #2 and #7 did not have documentation of the mother and father?s work phone number.
3. The record for child #3, #4 and #5 did not have documentation of the father?s work phone number.
4. Staff #1 confirmed that the above information was not documented in the children?s records.

Plan of Correction: 1. Gathered annual signature from a parent on 5/17/2024
2 & 3. The administration gathered all missing work numbers from listed families.
Going forward, the administration will ensure all paperwork and documentation are completed in full upon enrollment and scheduled annual and mid-point checks to ensure all information is correct and up to date, as well as utilizing the school`s Child File Tracking Workbook.

Standard #: 8VAC20-780-340-A
Description: Based on staff interviews and record review, the center did not ensure that when staff are supervising children, they shall always ensure their care and protection.
Evidence:

1. On 01/04/2024 at approximately 1:25 pm, staff #3 fed a 10-month-old child, child #8, another child?s bottle containing breast milk.
a. Administrative staff reported to the inspector, on 01/04/2024, that staff #3 had mistakenly given child #9?s bottle to child #8
2. Administrative staff and staff #4 stated that staff #3 fed approximately 3 ounces of child #9's bottle before noting that she was feeding child #8 the wrong bottle.
3. Administrative staff and staff #4 stated that both bottles were dated and labeled with each child?s full name and that staff #3 failed to follow center protocol for checking bottles before feeding.

Plan of Correction: All staff were given additional training on bottle feeding procedures on 1/5/2024, and 1/8/2024.
to help ensure this incident does not occur again, all staff who work in the Infant classroom are required to complete infant-specific training as well as following the Infant feeding Procedure, including saying aloud to the co-teachers in the classroom confirming the child?s name is correct.
