A renewal inspection was conducted on 06/10/2024 from approximately 10:20 a.m. to 5:40 p.m. There were 112 children present and 21 staff supervising. The inspector reviewed compliance in the areas of administration, qualifications and training, physical plant, staffing and supervision, programming, medication, special care and emergencies, and nutrition. Ten child (10) records and fourteen (14) staff record were reviewed. The information gathered during the inspection determined violations with applicable standards or law.
Please sign and complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days.
The plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventive measure(s).

Standard #: 8VAC20-780-270-A Description: Based on observation, the provider did not ensure that all areas of the center be maintained in a clean, safe, and operable condition.
Evidence:
The bathroom flooring in the early pre-school and pre-school rooms was observed to cracked and pulling away from the concrete floor creating a trip hazard for children and staff. Ceiling tiles in both bathrooms are broken and coming out of the metal framing track.Plan of Correction: Work order re-submitted.
Standard #: 8VAC20-780-500-B Description: Based on observation the provider failed to ensure that the diapering surface shall be cleaned with soap and sanitized after each use.
Evidence:
Licensing inspector observed a staff member in the early pre-school room change a child?s diaper. The staff cleaned the diapering surface after the diaper change; however, failed to spray the diapering surface with the disinfectant solution only and allow to air dry for a minimum of two minutes before starting the next diaper change.Plan of Correction: Staff were re-trained on proper diapering techniques.
