Comments:
A monitoring inspection was conducted on November 9, 2023 from approximately 12:30 PM to 4:00 PM. There were 39 children present in the center, ranging in ages from infant to preschool, with eight staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of six child records and seven staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the plan of correction and the date to be corrected for each violation listed on the violation notice and return it to the department within 5 business days from receipt. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s); 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures.

Standard #: 22.1-289.035-A
Description: Based on a review of staff records and interview, the center did not ensure that staff had the required repeat background checks every 5 years.

Evidence:
1, The record of staff #6 contained a fingerprint-based national criminal background check dated 04/16/18. The repeat check was required to be completed before 04/16/23.
2. A member of management acknowledged that the fingerprint-based national criminal background check had not been updated.

Standard #: 8VAC20-780-130-E
Description: Based on a review of children's records, the center did not ensure each record contained additional immunizations once every six months for children under the age of two years.

Evidence:
The record for child #2 (birth date: 10/24/21) had one immunization record documented at the time of enrollment on 02/06/23
2. The record for child #3 (birth date: 5/04/22) had one immunization record documented at the time of enrollment on 03/27/23.

Standard #: 8VAC20-780-270-A
Description: REPEAT VIOLATION
Based on observation, the center did not ensure areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.

Evidence:
1. On bus#1, the two front passenger seats had tears in the cushions of the seats. On the left side there were five tears and on the right side there were four tears.
2. On the preschool playground, next to the wooden fence surrounding the HVAC unit was a wire protruding above the ground, poses a tripping hazard. One water table had broken pieces and one large container was filled with approximately 5 inches of water, trash and a garden hose.

Standard #: 8VAC20-780-500-A
Description: Based on observation, the facility failed to follow all hand washing procedures, as required.

Evidence:
In the infant II classroom, a child was observed having a diaper change and afterwards staff moved the child to the table without washing the child's hands. The staff member began the next diaper change without washing her hands.
