Comments:
An unannounced monitoring inspection was conducted May 22, 2024 between the hours of 9:35am - 12:35pm. There were 117 children directly supervised by 16 staff. The physical plant, 5 staff records, 10 children?s records, medications, evacuation drills, injury reports, emergency supplies and policies were inspected. Children were observed participating in learning centers, group play and playing outside. There was an assortment of age appropriate materials for the children in care. Areas of non-compliance are identified in the Violation Notice. Please submit your plan of correction within five business days in order for it to be included on the public website.

Violations:
Standard #: 8VAC20-780-270-A
Description: Based on observation, the center did not ensure areas and equipment of the center, inside and outside, were maintained in a clean, safe, and operable condition.
Evidence:
1. On the Preschool playground, 2 of 3 tricycles had cracks in the seat and were missing handle bar plastic coverings.
2. On the Preschool playground play structure, rubber was peeling off the top stair, exposing rusty iron.
3. The Infant classroom had paint chipping off the wall next to the changing table.
4. The Toddler classroom had paint chipping off the lower wall in the classroom.
5. The Jr Toddler classroom had paint chipping off the wall near the adult sink.
6. The green carpet in the Two's classroom had fraying edges.

Plan of Correction: Retrain staff on reporting broken/damaged equipment so management can replace and/or submit work orders. Retraining will be conducted 6/13/24.

Description: Based on observation, hazardous substances such as cleaning materials were not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1. In the School age bathroom, 2 bottles of cleaning sprays were observed in an unlocked upper cabinet.
2. In the Toddler classroom, 2 bottles of cleaning sprays were observed in an unlocked upper cabinet.
3. In the Preschool bathroom, 2 bottles of cleaning spray were observed in and unlocked upper cabinet above the toilet.
4. In the Jr Toddler classroom, 2 bottles of cleaning spray were observed in the adult sink, next to the changing table.

Plan of Correction: Retrain staff on procedures for keeping cleaning materials locked up. Retraining will be conducted on 6/13/24.
