Comments:
A renewal inspection was conducted on May 29, 2024 from 10:30 am to 12:48 pm and on May 31 from 10:35 am to 12:40 pm. 60 children were present, ranging in ages from 9 months to 11 years with 14 staff supervising.

10 children?s records and 5 staff records were reviewed. Background checks for 5 officers and agents responsible for the facility were also reviewed.

The inspector reviewed compliance in the areas of administration, staff qualifications and training, physical plant, staffing and supervision, programs, special care and emergencies, special services, the license, the licensing process, and background checks.

During the inspection, children were observed participating in outdoor play that included utilizing playground equipment and participating in teacher-directed and self-directed activities. Staff were observed having positive interactions with the children. The center is equipped with toys and supplies and items were available to the children.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Please complete the plan of correction and date to be corrected for each violation cited on the violation notice and return it within 5 business days. Please specify how the deficient practice will be or has been corrected.

Your plan of correction must contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again; and 3) person(s) responsible for implementing each step and/or monitoring any preventative measures. Violations:
Standard #: 8VAC20-780-245-A
Description: Based on the review of one staff record (S3), and an interview with a center representative (CR1), the center failed to keep staff records as required.

Evidence:

There was no documentation in the record that S3 completed 16 hours of training appropriate to the age of children in care annually. There was documentation of 11 hours of training in 2023 for S3. In an exit interview on 5/31/24, CR1 acknowledged that 16 hours of annual training was not provided.

Plan of Correction: The director has reviewed the standard and going forward will maintain correct training hours.

Standard #: 8VAC20-780-270-A
Description: Based on observation and an interview with a center representative (CR1), the center failed to maintain play equipment in a safe and operable condition.

Evidence:

Two play structures on the playground were each observed to have multiple areas with rust where the thick, outer paint coating was worn away. Children were observed playing on one of the play structures. CR1 acknowledged that the children also play on the second play structure.

Plan of Correction: The director has notified the home office for repairs. The equipment will not be used until repaired.

Standard #: 8VAC20-780-340-D
Description: Based on observation, record review, and an interview with a center representative (CR1), the center failed to ensure that a program leader was regularly present in a grouping of children.

Evidence:

An assistant, (S5) was observed supervising a classroom of children ages 7 years to 8 years alone. The record of S5 documents that she has the qualifications to be an aide/assistant. In an interview, CR1 acknowledged that a program leader was not regularly present in that classroom.

Plan of Correction: The director will only have qualified leads in a classroom.

Standard #: 8VAC20-780-340-F
Description: Based on observation and an interview with a center representative (CR1), the center failed to maintain sight and sound supervision for two children under the age of 10 years.

Evidence:

Two children (Child #11 and Child #12) ages 5 years and 7 years were observed in the bathroom alone while the rest of their classroom was outside on the playground. Staff would not be able to hear the children inside the bathroom when they were on the playground. The bathroom was located inside the classroom. The classroom had a door that opened outside to the playground. CR1 acknowledged that staff were not using the system they developed to maintain supervision of children using the restroom when the rest of the class is outside.

Plan of Correction: The director will retrain the staff on safety and supervision. The director will also monitor to make sure procedures are followed.

Standard #: 8VAC20-780-500-B
Description: Based on observation and an interview with a center representative (CR1), the center failed to dispose of a soiled diaper in foot-operated storage system with a lid.

Evidence: In one classroom, this inspector observed an open, soiled diaper laying on top of other trash in a trash can without a lid. CR1 acknowledged that they needed to obtain a foot-operated, lidded storage system for that classroom.

Plan of Correction: A trashcan with a lid and step was placed in the class immediately.
