Comments:
An unannounced monitoring inspection was conducted beginning at 11:45 a.m. and ending at 1:45 p.m. with the director. After morning dismissal, there were 9 children in care ranging in age from 3 to 5 years of age under the care and supervision of 2 staff. Children were observed during morning dismissal, during lunch and preparing for outdoor time. Center documents and 10 staff records were reviewed.

Violations:
Standard #: 22VAC40-191-40-D-2
Description: Based on review of documentation and interview with staff, it was determined that the center did not have documentation of a central registry background check within 30 days of employment. Evidence: Staff #2, hire date 8/26/19, did not have documentation of a central registry background check in the record.

Plan of Correction: The background check office will be contacted to follow up on the status of the background check. If for some reason the office cannot locate the background check, a new background check will be completed.

Standard #: 63.2(17)-1716-B-3
Description: Based on review of documentation and interview with staff, the center did not have have the required policy regarding daily simple health screening, exclusion of sick children by a person trained to perform such screenings. Evidence: No staff had documentation of Daily Health Observation Training.

Plan of Correction: A daily health observation training will be scheduled as soon as possible.

Standard #: 63.2(17)-1720.1-A
Description: Based on review of documentation and interview with staff, it was determined that the center did not conducted repeat background checks every 5 years as required. Evidence: Staff #1, hired 7/2/07, most recent central registry background check was dated 8/24/2007. Staff #3, hired 8/23/07, most recent central registry background check was dated 7/9/2007. Staff #7, hired 10/1999, most recent central registry background check was dated 10/1/2004. Staff #9, hired 2/2000, most recent central registry background check was dated 8/17/2000.

Plan of Correction: Central registry background checks will be updated as soon as possible.
