Comments:
An unannounced complaint investigation was conducted on May 13, 2024. The center's video recording was observed and staff interviews were conducted in response to a complaint alleging a staff member roughly handling a child. The preponderance of evidence support s the allegations and the complaint has been deemed valid. Areas of non compliance are identified on the Violation Notice. Repeat violations were found and are identified within this report.

Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use staff names; list staff by positions only.

Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website.

Description: Based on video observation and staff interview, the staff to children ratio for 3 year olds up to school age was not maintained.
Evidence:
1. At approximately 12:00pm on 05/09/2024, the Pre-K classroom was observed having 15 children in care with one staff member supervising children. The staff to ratio for the Pre-K classroom is 1:10 ratio.
2.Staff #2 confirmed that the classroom was out of ratio for 15 minutes.

Plan of Correction: A staff meeting was held on 05/13/2024 with discussion of the importance of attendance, and teacher/child ratio. Moving forward, we will ensure all classrooms are in the correct ratio with management doing routine classroom attendance observations.

Description: Repeat Violation

Based on video observation, behavioral guidance was not constructive in nature, age and stage appropriate, and intended to redirect children in appropriate behaviors.
Evidence:
1. On 05/09/2024 at approximately 12:00pm, Child #1(4 years old) is observed in the block of the classroom after lunch. Staff #1 is observed grabbing Child #1'sleft arm and roughly placing them into chair. Child #1 gets up from their chair. Staff #1 is observed grabbing Child #1's arm, picking them up by their wrist, and roughly placing them into a chair.

Plan of Correction: Staff #1 is no longer an employee of Childtime Learning Center. A meeting with staff, discussion of appropriate handling of children, how to redirect children to explore other areas of the classroom when challenging behaviors arise.

Description: Repeat Violation

Based on video observation, staff used the forbidden action of roughly handling children in care.
Evidence:
1. At approximately 12:00pm on 05/09/2024, Staff #1 was observed sweeping the floor after lunchtime. Child #1(4 years old) is observed in the block area of the classroom. Staff #1 pulls Child #1's arm and places them into a chair. Child #1 is observed getting up from the chair and walking over to the wall area near the block area. Staff #1 is observed grabbing Child #1's left arm and guiding him towards a chair. Staff #1 is observed picking Child #1 up by the wrist and forcing them into a chair.

Plan of Correction: Staff were given 3 hour required training to refresh health/safety policies, challenging behaviors, and proper redirection of children.
