Violations:
Standard #: 8VAC20-780-330-B
Description: Based on observation, the center did not ensure where playground equipment is provided, resilient surfacing complied with the minimum safety standards to create a fall zone free of hazardous obstacles.

Evidence:
1. The climber with a slide located on the center's preschool playground did not have the required amount of resilient surfacing in the fall zone at the end of the slide shut. There must be at least six inches of resilient surfacing throughout the fall zone. The fall zone at the slide had approximately 1 inch of pea gravel. Areas of compacted dirt were visible where pea gravel was lacking.
2. The director stated the landscaping timbers had to be removed from around the equipment making it difficult to maintain the pea gravel for the fall zone.

Plan of Correction: Per the director: We will replace the landscape timbers and add additional pea gravel.
