22.1 BACKGROUND CHECKS CODE; CARBON MONOXIDE

Comments:
An unannounced monitoring inspection was conducted from 12:30 pm - 2:00 pm. There were 32 children in care being supervised by 6 staff. The children were observed resting. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and four staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations:
Standard #: 8VAC20-780-60-A
Description: Based on observation of five children's records it was determined that not all records were complete. Evidence: 1.) The records for child A (start date: 5/6/24), child B (start date: 5/6/24), child C (start date: 4/1/24), child D, (start date: 4/1/24), and child E (start date: 3/25/24) were missing the information of previous day care or schools attended. 2.) The records for child C and child D were missing the name and telephone number of their physician.

Plan of Correction: All information on the child's enrollment records were updated by the families.

Standard #: 8VAC20-780-80-A
Description: Based on observation of attendance records it was determined that the arrival and departure of each child was not documented as it occurred. Evidence: There were 18 children marked present on the attendance record of the 4's room, however only 15 children were physically present.

Plan of Correction: All staff have been retrained on attendance procedures.

Standard #: 8VAC20-780-270-A
Description: Based on observation of the playground it was determined that not all areas were maintained in safe and operable condition. Evidence: There were three tricycles observed on the preschool playground with cracked sharp edges on the seats. One tricycle was observed with a missing seat.

Plan of Correction: The tricycles were removed from the playground and new ones ordered.

Standard #: 8VAC20-780-340-A
Description: Based on observation it was determined that staff did not always ensure the care, protection, and guidance of children under their supervision. Evidence: The LI (Licensing Inspector) observed Staff A remove child A (Age: 13 months) out of their crib buy pulling the child up by their left arm and placing them onto the floor.

Plan of Correction: During the inspection, Staff A was holding a child while also trying to get the 13 month old child out of the crib. Staff A was retrained on how to pick up children.

Standard #: 8VAC20-780-510-L
Description: Based on observation it was determined that not all medication was kept locked to prevent access by children. Evidence: A medicated antiseptic cream was observed in a child's cubby in the three year old room. The medication was not locked as required.

Plan of Correction: There weren't any children in the 3 year old class during the inspection. It was determined that the family simply placed the medicated antiseptic cream in the child's cubby and did not notify anyone. Families will be reminded that medication is not to be placed in children's back packs or cubbies.
