Comments:
This inspection was conduced by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A renewal inspection was initiated on May 10, 2021 and concluded on May 11, 2021. The director was contacted by telephone to initiate the inspection. There were 45 children present 16 staff. The inspector emailed the provider a list of items required to complete the inspection. The inspector reviewed four children's records, nine staff records, and two agents records submitted by the facility to ensure documentation was complete.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the facility.

The Licensing inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care Programs.

Violations:
Standard #: 22VAC40-185-40-E
Description: Based on observation and review of the staff handbook, the center failed to ensure that the operational policies of the licensee ensuring the centers activities and services are maintained in compliance with the centers own policies and procedures that required by these standards.

Evidence:
1. Child 1M had an emergency medication that expired on 03/2021, it was not in the original packaging, and did not have the pharmacy label attached.
2. The staff handbook states, "the medication must be in its original packaging, it must be clearly labeled with the prescription with the child's name from the pharmacy, it must be a current prescription. Expired medications or medication no longer needed must be returned to the family and documented as returned.
3. Staff verified that the medication was not in the original box, did not have a pharmacy label, and was expired.

Plan of Correction: The parents were notified of the expired medication and will pick up the medication at pick-up time. There have been additional staff members trained in Medication Administration. Moving forward we will put in place a process where the medication will be reviewed by a MAT trained staff member and followed up with administration to make sure all required components are on file and completed. Staff will do periodic audits of all medication to ensure they are not expired.
