Comments:
A mandated monitoring inspection was conducted on May 13, 2024 from 10:50 A.M.-1:45 P.M. There were 45 children present, ranging in ages from six months to five years, with nine staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and nine staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Violations:
Standard #: 8VAC20-780-280-B
Description: Based on observation, the center failed to ensure that all hazardous materials were kept in a locked location.

Evidence:
1. In the shared restroom between Pre-K 1 and Pre-K 2 there was a swifter mop setting next to the toilet. The swifter mop had the cleaning solution attached to the mop.
2. The manufacturer's label states, "keep out of reach of children."
3. Children were observed using the restroom with the mop and cleaning solution accessible to the children.

Plan of Correction: A space in the locked closet has been designated for the swifter mop. Staff will return the mop to the secure location immediately after use. Before children use the bathroom staff will check the area for hazardous materials.

Standard #: 8VAC20-780-500-B
Description: Based on observation and interview, the center failed to ensure that the proper cleaning and sanitizing process was completed after diapering.

Evidence:
1. In the Infant Classroom, Staff #1, spayed the changing table with soap and water and immediately sprayed sanitizing disinfectant spray on top of the soap and water. The soap and water was not wiped off or cleaned before adding the sanitizing disinfectant spray.
2. Staff #1 verified that she did not wipe the changing table clean after spraying the soap and water.

Plan of Correction: Proper cleaning and diapering procedures were reviewed with staff assigned to the classroom. On 5/13 and 5/14 the director monitored diaper changes in all three changing classrooms. All staff will be review diaper changing and sanitizing procedures during the next scheduled staff meeting on 6/4/24.
