Comments:

An unannounced monitoring inspection was conducted on 06/04/2024 from 9:35 a.m. to 10:30 a.m. There were nine children present, ranging in ages from 17 months to four years old with one staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and one staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Description:	REPEAT VIOLATION Based on review of records, the provider failed to obtain documentation of a physical examination prior to a child?s attendance or within 30 days after the first day of attendance. Evidence: 1. Child #2?s start date was 03/11/2024. There was no physical examination on file for Child #2. 2. Child #4?s start date was 03/08/2024. There was no physical examination on file for Child #4.

Description:	Based on interview, the provider failed to ensure that a caregiver does not exceed 16 points. Evidence: 1. Upon arrival, the licensing inspector observed eight children in the care of one provider. The provider confirmed the ages of the children and that the total number of points was 17. 2. During the inspection, a ninth child arrived making the total number of points 19. 3. The provider confirmed that they were over 16 points and that they were the only caregiver.
