An unannounced subsidy health and safety inspection was initiated on 05/29/2024 with an on-site inspection of the program. The inspection was completed on 05/31/2024 with a review of 2 staff records at the Greenbrier North Family YMCA where staff records are maintained.
A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.'

Standard #: 8VAC20-790-600-F Description: Based on record review and staff interview, the vendor failed to ensure that staff who work directly with children shall annually complete the department's health and safety update course.
Evidence:
1. Staff 1, hire date 09/18/2021, lacks documentation of completion of the 3-hour annual preservice training course.
2. Administrative staff stated they could not locate any documentation to verify if staff 1 had completed the 3-hour subsidy training for 2023.Plan of Correction: Staff 1 will complete the annual subsidy training.
