Violations:
Standard #: 22.1-289.035-B-2
Description: Based on a review of six staff records and interview, the center did not ensure one staff member completed a national fingerprint check prior to the first day of employment.
Evidence:
1. The national fingerprint check for Staff #3, employed 02/13/2024, had results dated 03/01/2024.
2. Director confirmed that the fingerprint results were returned after the date of hire.

Plan of Correction: Ownership has reminded the management team of company policy, which requires all documentation, including but not limited to, background checks be completed and on file no later than the first day of employment. This policy ensures that newfy hired staff have completed all documentation required by the VDOE. Furthermore , management will conduct a lhorough review of all current staff records to ensure that all other staff members have completed required
documentation for employment. The management team will receive additional training for hlling to ensure compliance with all VDOE regulations involving employee records. Any future violations involving staff files will result in disciplinary action for management.

Standard #: 8VAC20-780-450-A
Description: Based on observation, the center did not ensure cots used by children other than infants during the designated rest period or during evening and overnight care shall have linens consisting of a top cover and a bottom cover or a one-piece covering which is open on three edges.
Evidence:
1. In the 2-year-old classroom, four out of 15 children did not have a bottom cover on their cot.
2. In the 3-year-old classroom, two out of nine children did not have a bottom cover on their cot.

Plan of Correction: Management will speak with the teachers in classrooms with a rest period
(excluding the infant classrooms) to remind them of the VDOE licensing requirement for cots to have both a top cover and a bottom cover or a
one-piece covering open on three edges during rest periods. In addition to reviewing this policy, management will provide any additional training and/or resources to teachers on proper rest period setup and bedding requirements (dependent upon individual teacher experience and needs).Furthermore, management will conduct daily checks during rest periods to ensure all children have the appropriate bedding on their cots and document any instances of non-compliance to follow up with teachers to address these issues. Lastly, families will receive a reminder about their obligation to provide a sheet and blanket for their child during the rest period on a weekly basis (teachers bag t them up on Fridays to be taken home and washed over the weekend), and management has purchase extra sheets for circumstance where students do not have them
