Comments:
An unannounced code compliance monitoring inspection was conducted on June 11, 2024 from approximately 10:30 am to 12:10 pm. There were 42 children in attendance and a total of 5 staff present. 4 staff records and 3 children?s records were viewed. All areas of the center were observed including classrooms, bathrooms, and the playground. The children were observed playing outside, listening to a story, playing in centers, eating lunch and taking a nap, while the licensing inspector was on-site.

Evacuation drills were conducted monthly with the most recent conducted on 05/02/24.

Violations were cited as a result of this inspection.

Please complete the plan of correction and ?date to be corrected? for each violation cited on the violation notice, sign, and return it to me within 5 business days from today, 06/20/2024. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standard(s), 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Violations:
Standard #: 22.1-289.031-A-3-d
Description: Based on observation, and interview, the center failed to follow the staff to children ratio of 1 staff to 10 children for children ages 36 months to five years.
Evidence: Classroom #2 had 15 children with 1 staff. The youngest child in classroom #2 was 3 years old. One additional staff was required. Administration confirmed that there was 1 staff with 15 children.

Plan of Correction: Moving forward, we will stop allowing children in the center if we have reached the staff to child ratio.

Standard #: 22.1-289.031-A-4
Description: Based on record review, the center failed to obtain a staff health report certified by a practicing physician or physician assistant that the staff member is free from any disability which would prevent them from caring for children under their supervision prior to employment.
Evidence: The record for staff #1, date of employment 10/20/2023, contained a staff health report dated 01/05/2024.

Plan of Correction: Moving forward, we will ensure we obtain a staff health report prior to employment.

Standard #: 22.1-289.035-B-4
Description: Repeat Violation
Based on record review, and interview, the center failed to obtain a copy of the results of a criminal history record information check and a search of the child abuse and neglect registry from any state in which the individual has resided in the preceding five years.
Evidence: The record for staff #1, date of employment 10/20/2023, indicated that staff #1 lived in another state outside of Virginia in the preceding five years prior to employment. The record had documentation of the criminal background check and central registry being requested in May of 2024. The results of the criminal background check must be received prior to employment. The central registry background check must be requested no later than 30 days after the first day of employment.

Plan of Correction: Moving forward, the criminal out of state background check will be obtained prior to employment.
