Comments:
An unannounced renewal inspection was conducted along with Licensing Inspector Christina Jones on 6/5/24 from 12:50 p.m. to 1:50 p.m. with the provider. There were six children in care, ranging in age from six months to five-years-old, for a total of 14 points. Children were observed napping. Three child records, two staff records, and one household member record were reviewed. The provider has current certification in CPR and First Aid. No medications were on site and the provider understands that they must obtain certification in Medication Administration Training (MAT) in order to give medication. The first aid kit, flashlight, weather band radio, and fire extinguisher were observed. The emergency drill log, attendance, and menu were also reviewed. Areas of non-compliance are identified in the Violation Notice. Violations:
Standard #: 8VAC20-800-180-A
Description: Based on a review of records and interview with the provider, the provider did not obtain subsequent tuberculosis (TB) screening, every two years from the date of the initial screening.

Evidence: The TB screening form for Staff # 1 had a tuberculosis screening on file dated 3/24/2022.

Plan of Correction: Will get a new TB test immediately

Standard #: 8VAC20-800-210-A
Description: Based on record review and interview with the provider, the provider did not ensure that all caregivers obtain a minimum of 16 clock hours of annual training.

Evidence: Staff #2 had documentation of 3 hours of annual training, Staff #2 confirmed there were no additional training hours completed for the year.

Plan of Correction: My assistant will complete their annual training immediately.

Standard #: 8VAC20-800-240-A
Description: Based on observation, the provider did not ensure the family day home was in a clean, safe, and operable condition.

Evidence: The outdoor plays space had small plastic pieces that were broken from toys throughout the playground on the ground, a piece of wood with four protruding rusty nails on the table, peeling paint on the concrete patio, peeling paint on the outdoor benches and table, cracked and broken wagon, cracked drum, cracked pan, three-wheeled trike with tire cracked down the middle.

Plan of Correction: We plan to clean up all broken pieces and put mulch down all around the playground to easily identify broken pieces or anything that is harmful to the children.

Standard #: 8VAC20-800-320-A
Description: Based on observation the provider did not ensure small electrical appliances were unplugged when not in use.

Evidence: In the kitchen a coffee machine was plugged in and not in use.

Plan of Correction: Will make sure anything not in use is not plugged in all around the house.

Standard #: 8VAC20-800-530-F
Description: Based on observation, the provider did not ensure that there was no soft bedding used under or around an infant.

Evidence: Child #1, six months old, was observed in a rocker with two blankets covering them.

Plan of Correction: Will make sure to have nothing in the cribs at all times.
