The renewal process was discussed with the Assistant Director.

Comments:
An unannounced renewal inspection was conducted today from 10:25am-2:15pm. There were 81 children directly supervised by 16 staff and all classrooms were in ratio. The physical plant, programming, menu, 7 staff records, 8 children?s records, 7 medications, evacuation drills, emergency supplies, and policies were inspected. Child were observed working on math, counting, and writing, playing Boggle, playing outside on the playground, eating lunch and completing an American flag art project. Infant care and feeding was observed. Lunch served today was a barbecue chicken sandwich with cucumbers, mixed fruit, and milk. Positive interactions between children and staff were observed during this inspection. The center was clean and organized. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-130-B
Description: Based on record review, the center has not obtained documentation of additional immunizations once every six months for all children under the age of two years. Evidence: 1. Child #3 is a child under the age of two years and immunizations were last updated on 8/20/2018. 2. Child #8 is a child under the age of two years and immunizations were last updated on 11/14/2018.

Plan of Correction: Requested updated vaccination records from families of both children. Both were provided on 7/2/2019.

Standard #: 22VAC40-185-160-C
Description: Based on review of 7 staff records, 1 staff record did not contain documentation of an updated tuberculosis screening or test completed within the last 2 years. Evidence: 1. Staff #6's most recent tuberculosis test on file was completed on 6/23/2017.

Plan of Correction: Staff's tuberculosis screening expired on 6/23/2019, during time off. Staff received updated TB screening upon returning to work on 7/8/2019.

Standard #: 22VAC40-185-280-B
Description: Based on observation of the physical plant, hazardous substances, such as cleaning materials, were not kept in a locked place using a safe locking method that prevents access by children. Evidence: 1. In the Youth Zone A classroom, a Resolve carpet cleaning bottle was located in an unlocked cabinet next to the door of the classroom. The cleaning material bottle was labeled "Keep out of reach of children".

Plan of Correction: Bottle of cleaner was immediately removed and thrown out.
